Once you make a HOTO flail, you will get the following rewards by wielding it: * Vex is difficult to obtain, you can only expect to drop randomly somewhere on Hell difficulty. * Pul only starts to drop in Act IV on Nightmare * Ko followed closely, it started to drop in Act II on Nightmare difficulty, but in the Hellforge on Nightmare or Hell, you have a higher chance to get it, or get it from The Countess on Hell. * Thul is the easiest to find in the HOTO runes, it starts to drop o the Act IV on Normal difficulty. You can find a 4-socket flail anywhere in Act IV Normal, or you can find a normal flail and add sockets using Horadric Cube, only flails are found in Act IV Normal, or you will have a chance to get 4 sockets later, or use Larzuk after you complete Siege of Harrogath quest in Act V. So add Ko rune, Vex rune, Pul rune and then Thul rune. You need to add the runes to the weapon in the same order, otherwise, you will not be able to get the Heart of the Oak. The base weapon of HOTO must have 4 sockets to spell out the runeword "KoVexPulThul".
